Comments: It is a rare spindle cell neoplasm that was initially reported in the pleura, mediastinum, and lungs. Recently, many extrathoracic locations, including kidney and retroperitoneal soft tissue, have been described.

Solitary Fibrous Tumor in Kidney

Comments: The tumor is composed of intersecting bundles of spindle-shaped cells with little atypia or mitotic activity. Some cases of solitary fibrous tumor have been associated with hypoglycemia due to abnormal secretion of insulin-like growth factors. Pathology International 1998; 48:47-52.

Leiomyosarcoma in Kidney

Comments: Leiomyosarcoma is the most common primary renal sarcoma. Most patients are beyond 40 years of age; M:F = 2:1. The tumor is composed of intersecting fascicles of smooth muscle cells.

Comments: This case shows minimal nuclear atypia. A mitotic figure is seen in the center of the image. The criteria for malignancy in smooth muscle neoplasms of kidney are not clearly defined. Large size, necrosis, nuclear pleomorphism, increased mitotic activity favor malignancy.

Comments: Primary MFH may arise in the renal parenchyma or in the renal capsule. More commonly, however, they arise in the retroperitoneal soft tissue and secondarily invade the kidney. They are large infiltrative tumors with a male predominance.

Comments: Most of MFH arising in the kidney are storiform-pleomorphic type as seen here. Distinction from sarcomatoid renal cell carcinoma may be difficult in some cases.

Angiosarcoma in Kidney

Comments: Primary renal angiosarcoma is an extremely rare tumor with less than 2 dozen cases reported in the literature (Arch Pathol Lab Med 1998 Oct; 122(10):929-35). This illustration shows anastomosing vascular channels with minimal atypia in the lining endothelium.

Comments: This illustration (same case as previous image) shows endothelial cells arranged in more compact sheets with abortive vascular spaces. Moderate cytologic atypia is seen.
