Post-brachytherapy biopsy showing benign prostatic acini with radiation-atypia at the lower left and a clusters of malignant acini in the middle and upper right of the core. In contrast to the atrophic but well-formed acini with little or no cytoplasm, the malignant acini are poorly-formed, often consisting of no more than two to three cells. They have abundant foamy cytoplasm and small dark nuclei. Occasional cells have enlarged hyperchromatic nuclei.
