Adult Granulosa Cell Tumor : Cording Pattern

Architectural Patterns in Adult Granulosa Cell Tumor (GCT) of the Ovary: Cording, gyriform, and watered-silk patterns resemble each other and consist of undulating ribbons or cords of tumor cells, separated by variable amounts of stroma.
Case History: This specimen is from a 39 y/o female who presented with acute onset of abdominal pain. Ultrasound revealed a large left ovarian mass. Salpingo-oophorectomy specimen consisted of a 12 x 10 x 8 cm ovary with a large adherent blood clot. The ovarian surface was smooth except for a 2 cm area of rupture. The diagnosis of adult GCT was confirmed on histologic examination. The clinical course of adult GCT is complicated by tumor rupture in 10% of cases. These patients present with acute abdominal pain and hemoperitoneum, as occurred in this case.
