Gross Pathology: Adult granulosa cell tumor (GCT) of the ovary is unilateral (>95% of cases), smooth, lobulated tumor with a predominantly solid or solid and cystic, yellow to tan-white cut surface. Yellow color is especially prominent in luteinized tumors.

The size varies from microscopic to >30 cm but the average size is around 10 cm. The consistency varies from soft (predominant component of granulosa cells) to firm (prominent stromal component). Hemorrhage and necrosis are not commonly seen and, when present, they can resemble high-grade ovarian carcinoma. About 10-15% of cases show gross evidence of rupture.
