Microscopic Features: Gonadoblastoma shows cellular nests composed of dysgerminoma-like immature large round germ cells with clear cytoplasm that are surrounded by smaller, dark sex cord cells (similar to Sertoli and granulosa cells). The nests are separated by hyalinized stroma which may contain steroid hormone-producing cells such as Leydig or lutein-like cells (two-thirds of cases), especially after puberty. These cells are responsible for endocrine manifestations; microscopic features continued in the next image.
