Metastatic renal cell carcinoma was strongly considered in the differential diagnosis of this bladder tumor. However, there were small scattered foci with morphology of conventional urothelial carcinoma. There was no past history of a renal mass and the imaging studies were negative. The immunohistochemical profile was as follows: CK7 strongly +ve, CK20 -ve, RCC -ve, Vimentin -ve. Final diagnosis : Urothelial Carcinoma, Clear Cell Variant.
