High-grade urothelial carcinoma invading muscularis propria. Such patients receive definitive therapy such as radical cystectomy, chemotherapy, or radiation therapy. Due to the treatment implications, the diagnosis of muscularis propria invasion should be made only in the presence of definitive evidence. In equivocal cases, it is appropriate to express uncertainty in the pathology report and recommend resampling.
