Microscopic Features of Leydig cell tumor (LCT): LCT is composed of uniform round or polygonal cells with abundant eosinophilic cytoplasm resembling mature Leydig cells. The nuclei are round, uniform, and variably-sized and have prominent central nucleoli. Cytologic atypia is minimal to absent and the mitotic activity is low.
Cytoplasmic lipofuscin pigment is seen in about 15% of post-pubertal cases. Some tumors have finely vacuolated or foamy cytoplasm (due to lipid accumulation) resembling zona fasciculata of adrenal cortex.
About 5% of cases show considerable cytologic atypia, increased mitotic activity and necrosis and they usually behave more aggressively.
