Ultrasound is the modality of choice for imaging testis suspected of harboring a neoplasm. Mature teratomas tend to be multicystic (as seen in this case) with heterogenous echoes in the fluid due to the mixture of sebaceous and mucinous material.
Immature teratomas tend to be more solid but still have variable echogenicity on account of hemorrhage and necrosis.
