Paraganglia are normal anatomic structures that may mimic adenocarcinoma. They are usually present outside the prostate adjacent to the posterolateral neurovascular bundles. Rarely, they are present within the outer peripheral zone of the prostate. They are encountered in needle biopsies (shown here) extremely uncommonly. They consists of a compact cluster of polygonal cells with clear or amphophilic cytoplasm (shown here). When seen in needle biopsies, paraganglia can mimic high-grade prostatic adenocarcinoma (Gleason score 4+4 and higher).
