Hepatocellular Carcinoma - Trabecular Pattern in Hepatitis C

The WHO describes several HCC histologic patterns: trabecular, acinar, solid, and scirrhous. The trabecular (sinusoidal) pattern is the most common and highly reminiscent of non-neoplastic liver parenchyma. The tumor architecture mimics that of normal liver but with thicker cell plates (>2 cells thick) and reduced or absent reticulin framework. This well-differentiated tumor comprises trabeculae of monomorphic polygonal cells with uniform round/oval nuclei and abundant granular eosinophilic cytoplasm. The trabeculae are separated by a network of vascular channels lined by endothelium.
