The incidence of high-grade PIN at our institution is about 8% (in several thousand prostate needle biopsies over a 10-yr period. This is similar to that reported in the literature. High-grade PIN does not elevate serum PSA. It cannot be detected by digital rectal examination or trans-rectal ultrasound of prostate. The diagnosis can only be made by examination of prostate tissue.
