Nucleus is typically central and round with variable degree of pleomorphism. Mitotic figures are rare, usually up to 2 MF/10 HPF. Sometimes cytoplasm can be clear due to glycogen content, numerous mitochondria or cytoplasmic vacuoles. The latter is referred to as clear cell leiomyoma, still a variant of epithelioid leiomyoma.
