Allergic fungal sinusitis (AFS): Gomori methenamine silver (GMS) stain of the thick mucus shows a few branching fungal hyphae. The causative agent in this case was Bipolaris. The fungal hyphae may be scarce and missed. The finding of "allergic mucin" is virtually diagnostic of AFS even in the absence of fungal identification by special stains.
