After extremities, head and neck region is the 2nd most common site of involvement and accounts for 5% to 10% of all synovial sarcomas. They occur in the paravertebral space and present as a parapharyngeal or retropharyngeal mass near carotid bifurcation.
Case History: The patient was a 15 year old male who presented with hemoptysis, dyspnea, and a two week history of dysphagia. This plain radiograph, lateral view, shows a 6 cm well-defined lobulated mass in the posterior pharynx, narrowing the airway. The final diagnosis following resection was Monophasic Synovial Sarcoma.
