Paget Disease of Bone (PDB) - Imaging Studies: Lumbar spine is one of the most commonly involved sites in PDB. The affected vertebrae initially show mixed lytic and sclerotic areas and in later stages a uniform increase in radiodensity. There may be loss of height as well as increase in the width of the vertebra, creating a picture frame appearance (better seen in the next image). The pelvic bones often show a combination of lytic areas and increased radiodensity. The disease bone has striated or honeycombed appearance.
Case History: This CT shows polyostotic Paget disease of bone in a 70 y/o male. There was involvement of several lower thoracic vertebra, L3, right hemipelvis, sacrum, right knee and left proximal femur. Note the coarse trabeculation in the affected areas (arrows).
