Primary CNS lymphomas of diffuse large B-cell type are high-grade lesions that need to be distinguished from secondary CNS lymphoma or leukemia, metastatic carcinoma, small cell glioblastoma, oligodendroglioma, primitive neuroectodermal tumor, and melanoma.

Sampling errors or prior steroid therapy can distort the findings in small biopsies and lead to mistaken diagnosis of infection, vasculitis, or demyelinating disorders.
