Respiratory bronchiolitis - a common finding in smoker's lungs - is usually present in cases of pulmonary Langerhans cell histiocytosis (LCH) as most patients are smokers.

It is characterized by the presence of pigmented macrophages in air spaces adjacent to LCH infiltrates. Other smoking-related changes may be present in the adjacent parenchyma.
