Syringotropic mycosis fungoides: The lymphoid infiltrate is centered around disorganized, hyperplastic sweat gland epithelium. Rare atypical lymphoid cells with irregular nuclei are present.
Cases of syringotropic MF that arise in a background of classic MF show atypical lymphoid cells infiltrating the epidermis (epidermotropism). Immunostains for CD2, CD3, CD4, and CD45RO are positive in the atypical lymphoid cells. CD7 may be negative. T-cell receptor gene rearrangement studies (Southern Blot analysis or PCR) confirm clonal expansion.
