Mycosis Fungoides - Patch Stage: The microscopic findings in early lesions of the patch stage are subtle and may be easily missed. There are rare or no atypical cells and the lymphocytic infiltrate can mimic other inflammatory dermatoses. The recognition of MF is based more on identifying the pattern of infiltration rather than the presence of atypical cells with cerebriform nuclei.
The earliest patches show perivascular infiltrates of small lymphocytes around superficial venous plexuses. A few cells may be scattered interstitially within the papillary dermis. The propensity of lymphocytes to colonize epidermis in MF is known as epidermotropism. The lymphocytes can palisade along the dermal-epidermal junction and also collect within intraepidermal vesicles (Pautrier abscess).
