Cutaneous Marginal Zone Lymphoma (CMZL) - Morphology (continued) : Plasmacytic differentiation is common in CMZL. Monotypic plasma cells are usually found at the periphery of the infiltrate and in the superficial dermis beneath the epidermis. Rare CMZL cases with pure population of plasma cells were previously classified as primary cutaneous plasmacytoma.
This image shows numerous lymphoplasmacytoid cells and mature plasma cells. Dutcher bodies (PAS+ eosinophilic intranuclear inclusions) may be present. Monocytoid B cells with more abundant clear cytoplasm are prominent in some cases. In addition, there are variable numbers of larger transformed cells (centroblasts, immunoblasts), histiocytes, and eosinophils. Rare cases show multinucleated giant cells and granulomas.
