Splenic Involvement in Hodgkin Lymphoma: Spleen involvement is suggested by palpable splenomegaly and can be confirmed by imaging studies such as fludeoxyglucose - positron emission tomography (FDG-PET) or CT. Findings on FDG-PET include diffuse uptake, solitary mass, multiple large nodules, or miliary lesions.
Spleens involved by Hodgkin lymphoma usually weigh > 400 grams. The disease may be focal and some cases have small inconspicuous nodules that requres meticulous sectioning.
Grossly, there may be a single or a few large nodules (shown here) or multiple smaller nodules with a miliary distribution (next image). The number of tumor nodules has prognostic implications and should be noted (more specifically, it should be mentioned whether there are five or more nodules).
