Acinic cell carcinoma is an indolent tumor that is usually cured by complete resection. Superficial parotidectomy is done when the tumor involves the superficial lobe of the parotid. When the deep lobe is involved, the procedure of choice is total parotidectomy. Neck dissection is not indicated unless nodes are clinically involved. For incompletely resected tumor or metastatic disease, radiation therapy may be used.
The recurrence rate is about 35%. The tumor metastasizes in about 15% of patients. Most recurrences and metastases are seen within 5 years of treatment but may be delayed by decades. The 5-yr and 10-yr survival rates are around 90% and 85% respectively.
