Aggressive Angiomyxoma - Microscopic Features: It is hypocellular and a highly vascular lesion. It consists of a sparse population of primitive round or stellate mesenchymal cells in an edematous stroma. There is no cytologic atypia or increased mitotic activity. Large dilated blood vessels with hyalinized adventitia are frequent.
The lesional cells are positive for desmin, smooth muscle actin (50% of cases), CD34 (50%), estrogen and progesterone receptors (100%), and cytokeratin AE1/AE3 (25%).
The differential diagnosis consists of fibroepithelial polyp, angiomyofibroblastoma, cellular angiofibroma, and massive vulvar edema.
