Cytologic atypia in Ovarian Steroid Cell Tumors: Most steroid cell tumors have uniform round nuclei lacking cytologic atypia. Cytologic atypia, especially grade 2-3 atypia when present, is often associated with malignant behavior. This image shows moderate (grade 2) nuclear atypia.

About 30-40% of ovarian steroid cell tumors are malignant. Features associated with malignancy include: grade 2-3 cytologic atypia, ≥2 mitoses/10HPF, hemorrhage, necrosis (see next image), size >7cm, and patient age >51 years.
