High-grade papillary urothelial carcinomas readily shed tumor cells from their papillary fronds. In rare cases, the neoplastic epithelium may be denuded almost entirely leaving behind naked fibrovascular cores as seen here. Careful search revealed small foci of high-grade cells in rare papillary fronds in this transurethral resection allowing the diagnosis of non-invasive high-grade papillary urothelial carcinoma to be made.
