Office based cystoscopy with a flexible cystoscope is an easy, convenient way to evaluate urothelium. Papillary tumors stand out against a background of smooth urothelium. It is not possible to predict the pathologic diagnosis based on endoscopic appearance; however, if the tumor is sessile or necrotic, it generally indicates invasive, high-grade disease.
