Colonic adenocarcinomas may involve urinary bladder by direct extension, invade the mucosal surface, and even mimic primary villous adenomas. Extension from an intestinal malignancy should be ruled out clinically when considering the diagnosis of a primary villous adenoma of the bladder in a small biopsy specimen.
