The paratesticular region and sometimes the testis proper are locations for primary tumors that resemble ovarian-type epithelial tumors (serous, mucinous, endometrioid, Brenner, and clear cell types). The example shown here is a serous cystadenofibroma that was centered around tunica albuginea.

The patient was a 47 y/o man with a long-standing history of an intratesticular cyst associated with intermittent pain and discomfort. Testicular ultrasound showed a 3 cm hypoechoic, septated cyst. The orchiectomy specimen showed multiple tubules and cystic spaces in a thickened tunica albuginea ranging in size from microscopic to 0.8 cm. Atrophic seminiferous tubules can be seen in the lower left corner of this image.
