Malignancy in Sertoli Cell Tumor: Malignant behavior is more likely in Sertoli cell tumors (SCT) than Leydig cell tumors (LCT) of the testis : 10-35% of cases for SCT vs 5% for LCT.

Unlike with LCT, malignant SCT cases have been reported in children. Gynecomastia is more frequently seen with malignant SCT.

Malignant behavior is difficult to predict from morphology. The features the correlate with greater risk of malignancy include: size > 5 cm, cytologic atypia, infiltrating borders, predominant diffuse growth pattern, > 5 mitoses/10 HPF, Ki-67 proliferation index > 30%, vascular invasion, and necrosis. None of those features are seen in this image.
