Testicular lymphomas can mimic seminomas grossly; however, they are more likely to show extratesticular spread. Lymphomas penetrate through tunica albuginea in about 50% of cases. Epididymis is involved in the majority of cases. Rare cases show spermatic cord involvement.
This orchiectomy specimen shows complete replacement of testicular parenchyma by a diffuse, yellow-tan tumor. The knob-like structure at the top is the head of epididymis that is infiltrated by the tumor. The final diagnosis was : Primary Diffuse Large B-cell Lymphoma of the Testis.
