Benign prostatic hyperplasia is subclassified based on the relative proportions of epithelial and stromal components. One of the earliest changes is the formation of pure stromal nodules of loose mesenchyme as shown here. While such nodules are occasionally encountered in prostate needle biopsies as shown here, the diagnosis of prostatic hyperplasia (or nodular hyperplasia) should not be rendered in biopsy specimens. They should simply be signed out as benign prostatic tissue.
