Basal cell hyperplasia in needle biopsies usually consists of solid nests, a feature that is rarely encountered in acinar adenocarcinoma. The solid appearance is secondary to multilayering of basal cells. The basal cells have round or oval crowded nuclei with punctate nucleoli and scant cytoplasm. By contrast, the glands in adenocarcinoma would show single layer of acinar cells, abundant eosinophilic cytoplasm and prominent nucleoli.
