One may occasionally see spindling of tumor cells in papillary renal cell carcinoma. Such cases should be distinguished from sarcomatoid renal cell carcinoma (which would show greater degree of cytologic atypia) and mucinous tubular and spindle cell carcinoma (which would show mucinous stroma that is lacking in papillary renal cell carcinoma).
