The cysts in multilocular cystic renal neoplasm of low malignant potential are lined by a single layer of small bland clear cells without nucleoli, typical of WHO/ISUP Grade grade 1 or 2 nuclei.
The presence of grade 3 or 4 nuclei, expansile nodules of tumor cells, necrosis, or sarcomatous change is incompatible with the diagnosis of multilocular cystic renal neoplasm of LMP.
