Intraductal papillary mucinous neoplasms (IPMNs), pancreatobiliary-type are the least common subtype and usually involve the main pancreatic duct. They consists of complex, branching and interconnecting papillary structures lined by cuboidal cells with amphophilic cytoplasm. There is nuclear enlargement with prominent nucleoli. Most lesions are high-grade (as shown in this case).
