The histologic hallmark of acute fatty liver of pregnancy is microvesicular steatosis. This high power view shows relative sparing of periportal hepatocytes. A few mononuclear inflammatory cells are scattered throughout the lobules. Many cases show extramedullary hematopoiesis with prominent megakaryocytes and erythroid precursors.
