Differential Diagnosis (continued from the previous 3 images): Metaplastic changes can be mistaken for gallbladder carcinoma. The most common type is antral or pyloric metaplasia (shown here). The metaplastic glands may have a pseudoinfiltrative appearance and can impinge upon nerves and mimic perineural invasion. Features favoring a benign metaplasia include lobular configuration, lack of cytologic atypia, and the presence of glands within the mucosa or superficial muscular layers.
