Acute Acalculous Cholecystitis: By definition, the inflamed gallbladder has no gallstones. The gallbladder is enlarged and shows serosal congestion. The appearance of mucosa ranges from normal to hyperemic to widespread ulceration and necrosis (as shown here). Microscopically, the mucosa shows bile infiltration, vascular congestion, neutrophilic and mononuclear cell infiltrate, and dilatation of lymphatics. When the etiology is drug-induced injury to gallbladder mucosa, the inflammatory infiltrate shows a predominance of eosinophils.
