The vast majority of esophageal adenocarcinomas arise in the setting of Barrett esophagus (as shown here). In the presence of high-grade dysplasia, the risk of developing invasive carcinoma in Barrett esophagus increases 30 to 40 times that seen in the general population. However, it is also true that most patients with Barrett esophagus don't develop malignant tumors.
