Large Cell Transformation in Mycosis Fungoides (MF): The transformed large cells in this case had a pleomorphic appearance with irregular nuclei and prominent nucleoli. Mitotic activity was brisk.

CD30 is expressed in 30-50% of cases of large cell transformation in MF. Such cases can be difficult to distinguish from cutaneous anaplastic large cell lymphoma. The distinction may only be possible on clinical grounds. The presence of typical patches and plaques at other sites would support MF.
