Large Cell Transformation in Mycosis Fungoides (MF): The transformed cells are four or more times the size of a small lymphocytes and have a blast-like appearance. They have large hyperchromatic or vesicular nuclei, prominent nucleoli and abundant cytoplasm. They may resemble anaplastic large cell lymphoma (ALCL) and, in some cases, the correct diagnosis of MF can only be made if there are typical patches and plaques elsewhere on the body.
