Fuhrman grade 3 nuclei, as demonstrated in this image, may be patchy, show moderate pleomorphism and large nucleoli. Some nuclei are three times the size of others, however, no bizarre forms or sarcomatoid transformation are seen which would qualify for Fuhrman nuclear grade 4.
