Comments:
Prepubertal-type teratomas, including epidermoid and dermoid cysts are biologically and clinically distinct from postpubertal-type teratomas and behave in a benign fashion. When the diagnosis is certain preoperatively, a conservative (testis-sparing) approach is recommended.

This case was diagnosed on testicular ultrasound (see next image for characteristic ultrasound findings) and the tumor was removed piecemeal instead of by radical orchiectomy. The specimen shows fragments of cyst wall as well as its contents.
