Comments:
The image shows microvesicular steatosis, hypertrophied Kupffer cells with lipofuscin pigment, and mononuclear infiltrate in the lobules in a case of acute fatty liver of pregnancy. Since the pathogenesis involves a fetal metabolic deficiency causing long-chain fatty acid overload in maternal circulation putting strain on the liver, the mainstay for therapy is early delivery/termination of the pregnancy and supportive care.
