High-grade prostatic intraepithelial neoplasia (HGPIN) is a proliferative lesion that is precursor to some prostatic carcinomas. It consists of intermediate to large size glands with benign architecture but lined by cytologically atypical cells. High-grade PIN appears basophilic at low magnification due to nuclear stratification, hyperchromasia, and enlargement. The image shows adenocarcinoma (top left and bottom right) adjacent to high-grade PIN glands.
