Comments:
The differential diagnosis of Whipple disease includes mycobacterium tuberculosis granulomas (PAS-negative; Acid-fast positive) and mycobacterium avium-intracellulare granulomas (PAS-positive and Acid-fast positive), lysosomal storage diseases (Gaucher, Fabry), and abdominal lymphomas.

The image shows aggregates of foamy macrophages in a mesenteric lymph node in Whipple disease.
