Gross Pathology: Regressed germ cell tumors (GCTs) of the testis can appear as tan-white, well-circumscribed, nodular (or multinodular) areas of scarring. Some cases show ill-defined, stellate, or band-like scars. Scars may be very small and no residual viable tumor may be found even after thorough sampling. Residual gross viable tumor is seen in a small percentage of cases.

Case History: This orchiectomy specimen is from a 50 year old male who presented with testicular enlargement of about 1 years' duration. Serum tumor markers were negative. Ultrasound showed multiple, ill-defined hypoechoic areas.

The specimen shows a large ill-defined tan-white scar. Histologic features were consistent with regressed GCT. The cystic area (in the right half) was residual teratoma lined by simple columnar epithelium.
