At high power, fetal epithelial-type hepatoblastoma may show mild cytologic atypia, with enlarged nuclei and prominent nucleoli, as seen in reactive hepatocytes and well-differentiated hepatocellular carcinomas. Some tumor cells appear lighter due to cytoplasmic lipid and glycogen, while others have eosinophilic cytoplasm, resembling non-neoplastic adult hepatocytes.
