Insulinoma - Microscopic Features (continued from previous image): In the appropriate clinical setting, immunohistochemical demonstration of insulin is not necessary for the diagnosis of a solitary insulinoma. In case of multiple PanNETs and familial insulinomatosis, it is important to identify which of the many tumors is producing insulin. It is often not the largest tumor.

About 50% of insulinomas have scattered tumor cells producing other hormones, including somatostatin, glucagon, and PP. This feature is more common in malignant insulinomas. Image courtesy of: Dr. Aatur Singhi (@PancPathologist); used with permission.
